How can I retrieve my lost ID and SS card for NY? - ny id cards
My friend has his ID and SS lost cards. In New York for a need for another, so that we have a very hard time. Who knows what can we do? Thank you:)

For the SS card, all you have to do is fill out the form and send it to your local office of the SS only a birth certificate and other forms of identification such as a health insurance card, employee ID card, passport, etc .
